Insurance Information
Contracted Health Plans:
- We are contracted providers for several PPO plans and handle all insurance processing for covered patients.
- Please make sure the doctor you are seeing is on your plan.
- Co-payments and deductibles are due at the time of service. Please be aware that co-payments are usually applicable to office visits only. Any procedure done in the office is usually processed differently.
Medicare:
- Assignment is accepted.
- Please remember that we accept assignment which means we accept what medicare allows. They only pay 80% of the allowable amount and they have an annual deductible of $131.
- We will bill one supplemental plan for you.
Other Health Plans:
- We are not members of Cigna, United Healthcare, Aetna, Healthnet and many other plans.
- Payment is requested at the time of service.
- Arrangements can be made for billing purposes.
Non-insured:
- Payment is required at the time of service.
- We accept cash, check, visa or mastercard.
