Payment Policies
Our office only accepts cash or check. If your insurance company requires a referral please make sure to bring it with you at the time of service, otherwise we will request that you reschedule your appointment. Copays must be paid at time of service, otherwise a $25.00 billing fee will be applied to your account.
Cancellations must be made 24 hours prior to your appointment, otherwise a $25.00 broken appointment fee will be applied to your account. Cosmetic procedures must be paid in full at time of service. If payment is not made on the day of service, an additional $25 charge will be added to the bill.
