Please print and fill out the following forms prior to your appointment. For patients being seen for school related problems i.e. ADHD etc please print two copies of the VICHQ form and have teachers fill one set of forms and parents to fill out one set.For other neurology problems please fill only the first four forms..The short vanderbilt form and medication follow up form should be filled by teachers at follow up visits.
